What is an HRA
The HRA provided through Paylocity is funded by Big Brand Tire to help you pay part of your medical plan’s deductible. Only an employer can fund an HRA. You cannot add to the account. You can be reimbursed for deductible expenses as you incur them, up to the amount Big Brand Tire contributes.
Details
Big Brand Tire will contribute to your HRA if you enroll in the Core PPO Plan. For this plan year, the company’s HRA contribution will be $1,200 if you enroll as an employee only, or $2,400 if you cover one or more dependents. The full amount will be available to reimburse deductible expenses on the first day of the plan year, January 1.

What Can the HRA Pay For?
Big Brand Tire’s HRA is available to reimburse you for covered services that are subject to the calendar year deductible, including office visits, hospital stays, urgent care, prescription drugs, and more.
How Do I Access My HRA Funds?
You can access your HRA to be reimbursed for qualified expenses by completing an electronic claim or by submitting a paper claim.
Electronic Claim
Electronic Claim: You can submit an electronic claim by signing into your account at https://bat.paylocity.com or the mobile app. You will complete an online claim form and upload an e-copy of your Anthem EOB (Explanation of Benefits) and/or itemized receipt.
Paper Claim
Fax or email a copy of your paper claim form and your EOB to Paylocity. Paper claim forms are available online or by mail when you call 800-631-3539.
Required Documentation
A copy of your EOB (Explanation of Benefits) from you insurance company is required to approve a claim for reimbursement.
* Reimbursement is limited to the annual contribution made by Big Brand Tire and is available via a HRA debit card.
